Please complete this form for reimbursement. Make sure to upload all applicable documents, via the pink “Upload Here” Dropbox button. If you would prefer the paper copy, please download it here and turn in to your campus office.

  • Must be a PTO member to apply

  • Funds are not transferable between staff

  • Tax cannot be reimbursed

  • Can be turned in throughout the school year; Deadline: Friday, May 10, 2024

  • Please remember to submit:

    • Receipt(s) for reimbursement

    • Invoice(s) for vendors

    • Proposal for fund request
